Matthew Horne

Matthew Horne

Matthew Horne, PT, CSCS is a 1998 graduate of the University of Vermont. He received his Strength and Conditioning Specialist Certificate in 2002, and he & Tracy are the owners of Addison Wellness, Inc. Matt’s professional experiences have included inpatient acute care, home health and advanced work in outpatient orthopedic and sports rehabilitation. Matt currently lives in Middlebury with his wife and two daughters.

Tracy Horne

Tracy Horne

Tracy Horne, PT is a 1998 graduate from University of Vermont. Her clinical experience work is in various settings. She has extensive training in postural repositioning, maximizing biomechanical function and recovery. She lives in Middlebury with her husband and two daughters. She enjoys being outdoors, reading and spending time with family and friends.

Lisa Perkins

Lisa Perkins, Office Manager & Billing Specialist graduated with a BA in History from Castleton State College in 1995 and received her certificate in Herbal Medicine from Sage Mountain in 2001. Her interest in natural therapies led her to administrative and billing work in chiropractic, naturopathic and physical therapy offices gaining 13 years experience in those fields. Lisa joined the Wells PT team in 2009. She lives with her husband and two daughters in Monkton, and in her spare time enjoys hiking, camping, biking, gardening and ghost hunting.

Lauren Peterson

Lauren Peterson, Clinic Assistant, Receptionist & Marketing Assistant joined the Wells Physical Therapy team in December 2013. Originally from San Diego California, Lauren has enjoyed the Vermont Lifestyle for the past six years. She has grown to love and appreciate all that a small town community has to offer, especially that it gives her the opportunity to establish friendly and caring relationships with our patients. When she’s not at work, Lauren enjoys her time with her adorable daughter Lilia exploring the Vermont countryside.
